Sand Dollar Dental Membership Plan

To save our patients money, we have created a plan that provides high-quality dental care with an emphasis on prophylaxis treatment. This plan is designed to improve oral healthcare in the community by allowing better opportunities for access to care. With an amazing discount on the purchase of the package, there is also a great discount on services outside the preventive services we offer, such as:

• Crowns
• Fillings
• Whitening
• Root canal treatments
• Deep cleanings
• Extractions

This is not an insurance plan; it is a dental cost reduction plan offered to those who do not have any other dental plans and is only accepted here at Sand Dollar Dental, Navarre.

• One person $399.00, that’s a savings of 33% off the services provided.
• Dual enrollment $780.00, that’s a savings of 35% off the services provided.
• For a child plan as a stand-alone or additional member below the age of 13, the plan is $275. At the age of 14, they will be recognized as an adult based on their dentition.

Coverage Will Include

• 2 exams per year with all necessary X-rays D0150 or D0120 with either D0210 or D0274
• 2 prophylaxis per year D1110
• 1 fluoride per year D1208
• BURST electric toothbrush
• Emergency visit with X-rays D0140 and D0220
• Restorative and endodontic services will receive a 20% reduction from the regular fee.
• Ortho services will receive a 10% reduction.
• Total payment for services rendered must be paid at the time of service. If not paid in full at the time of service, the reduced rate will be void.
• Missed appointments or cancellations without a 24-hour notice are not eligible for the price reduction.
• Sand Dollar Dental Too will reassess the yearly rate and adjust as needed.
• The entire cost of the program must be paid at the time of sign-up.
• This program cannot be purchased or used with any other dental plan, insurance, or discount program. No medical plans, workers’ compensation claims, auto claims, home claims, or commercial insurance claims may be used.
• If CareCredit is used, the reduced rate will then be 10% due to fees charged by the financier.
• Payment for any services that are reduced will be paid at the time services are rendered.
• No refunds will be given once the program is purchased. This program is NON-REFUNDABLE.
• If family members are going to be on the program, they will need to be purchased at the same time or at renewal. They cannot be added throughout the yearly period.
• Each plan is for individual use and is non-transferable.
• A $75 reinstatement fee will be charged if the program lapses.
• This is an in-house membership plan and not an insurance policy.
• This in-house plan will only be accepted at Sand Dollar Dental Too Inc. and cannot be used at any other provider.
• If SRP deep cleaning is determined to be the treatment needed for the patient, the deep cleaning fee will be reduced by 20%. After SRP, it is required to have 4 maintenance appointments per year. 2 of those are provided in the plan, and the other 2 will see a reduction of 20%.
• Botox is excluded from the membership program.

How the Program Works

• You call to make an appointment and inform us that you are interested in the Sand Dollar Dental membership program, or discuss the program with the front office coordinator.
• We will then take all your information to get you into the system.
• Payment is then required, all up front, as there will be no availability to make payments on the program.
• After payment has been made, you will then be scheduled for a comprehensive examination and a full set of X-rays with Dr. Graves.
• At that exam, Dr. Graves will derive a treatment plan for you and determine what type of periodontal therapy will be required for you.
• After this has been completed, you will then see the treatment plan coordinator to go over the findings and get you scheduled for your periodontal therapy and start any dental work Dr. Graves has planned for you.
